Housing Stabilization Services

Housing Stabilization Services (HSS) were created by the Minnesota Medical Assistance program as a benefit to help people with disabilities, including mental illness and substance use disorder, and seniors find and keep housing. At Independent Living Services, our Housing Stabilization Services follow a Housing Focused Person-Centered Plan.

Developing, updating and modifying the housing support and crisis/safety plan

Preventing and early identification of behaviors that may jeopardize continued housing

Transportation with the person receiving services present and discussing housing related issues

Education on roles, rights, and responsibilities of the tenant and landlord

Help in maintaining key relationships within the housing community
Promote housing retention and assist with the housing recertification process
Advocating with community resources to prevent eviction
Training on being a good tenant, lease compliance, and household management

Note: Housing Stabilization Services do not pay for a person’s housing.

HSS Requirements

At Independent Living Services of Central Minnesota, we offer HSS services to individuals that meet the following requirements:

HSS is not for those receiving similar services from other programs like Moving Home MN, Assertive Community Treatment, Housing Access Coordination, or Relocation Service Coordination.
  • Must Be 18 Years Or Older
  • Receive Medical Assistance
  • Experiencing Housing Instability
  • Currently Experiencing Homelessness
  • At Risk Of Becoming Homeless
  • Transitioning From A Licensed Setting
  • Eligible For Ma Waiver Services

Needs Assistance Due To Their Mental Health Condition To:

  • Communicate
  • Manage Challenging Behaviors
  • Make Decisions
  • Mobility
